Hormonal Acne: Causes, Signs, and Care Options

What people mean by hormonal acne, the signs that suggest it, everyday care, and the treatments a doctor may offer.

Hormonal acne is an everyday name for breakouts that follow changes in hormone levels. It is not a separate medical diagnosis, because hormones play a part in almost all acne. The term is most often used for adult women whose acne flares at certain times of the month or of life.

How hormones cause breakouts

Hormones called androgens tell the oil glands in the skin to make more oil. More oil makes it easier for pores to clog. Androgen levels and the skin's response to them change during puberty, across the menstrual cycle, in pregnancy, around menopause and when starting or stopping some types of birth control.

Common signs

No single sign proves that acne is hormonal, but this pattern is typical:

  • Breakouts on the lower face: the chin, jawline and sometimes the neck.
  • Flares in the week before a period.
  • Deep, tender bumps as well as surface pimples.
  • Acne that begins or returns in the twenties, thirties or later.
  • Breakouts that come back in the same places.

Times when it often appears

Life stageWhat changes
PubertyAndrogen levels rise and oil glands become active
Menstrual cycleHormone shifts before a period can trigger flares
PregnancyAcne may improve or worsen, often in the first months
After stopping birth control pillsOil production can increase for a time
PerimenopauseFalling estrogen changes the hormone balance

Signs that need a medical check

See a doctor if acne appears together with irregular or absent periods, new hair growth on the face or body, thinning hair on the scalp, or rapid weight change. These can be signs of a hormone condition such as polycystic ovary syndrome, which needs proper diagnosis.

Everyday skin care

Skincare cannot change hormone levels, but it helps to control their effect on pores.

  1. Cleanse gently twice a day.
  2. Use a proven acne ingredient such as a retinoid, benzoyl peroxide, salicylic acid or azelaic acid.
  3. Moisturize with a light, non-comedogenic product.
  4. Wear sunscreen daily. It helps to stop marks from darkening.

Do not pick deep bumps. They have no opening, and squeezing increases the chance of scars. Skincare Routine for Acne-Prone Skin sets out a full routine.

Treatments a doctor may suggest

When over-the-counter products are not enough, a doctor or dermatologist can discuss prescription options. These may include prescription-strength creams or gels, certain combined contraceptive pills, a medicine called spironolactone that reduces the effect of androgens on the skin, courses of antibiotics, or isotretinoin for severe acne. Each has benefits, side effects and rules about use in pregnancy, so the choice needs a medical consultation.

What to expect

Hormone-related acne tends to improve slowly. Most treatments need two to three months before a fair judgment can be made. Keeping a simple record of breakouts and your cycle for a few months can show a pattern and is useful to take to an appointment.

Frequently asked questions

Is hormonal acne only a problem for women?

Hormones drive acne in everyone, including teenage boys and men. The phrase is mostly used for women because their breakouts often follow the menstrual cycle or life stages such as pregnancy and menopause.

Can a blood test show whether my acne is hormonal?

Most people with acne have hormone levels in the normal range, so tests are not routinely needed. A doctor may order them if there are other signs of a hormone condition.

Will hormonal acne go away by itself?

It can ease with time, but it may last for years. Treatment is worth seeking if acne is painful, leaves marks or affects how you feel.
